The IU Northwest Campus Health and Wellness Center Clinic is made possible through the College of Health and Human Services. It is an organization of health care professionals who are committed to improving the health and well-being of the IU Northwest campus community. We seek to do this through the delivery of high quality, affordable health care and through health promotion and illness prevention activities that provide students, faculty, and staff with the tools they need to live a healthy lifestyle.
If you would like to save time and complete new patient intake paperwork prior to your appointment at the Health Clinic, click on the links below and fill out the forms online. After you're finished, print them and bring them with you to your appointment.
Nutritional History Questionnaire (if seeing the Dietitian)
You must bring your IUN ID card, and if you have insurance through IU, please bring your insurance card as well. Call us at 980-7250 for more information, or if you cannot make it to the following walk-in clinic times and would like to instead schedule your appointment.
On the following days and times, you may walk in for your flu shot; no appointments are necessary.
